Extended Day Care Forms for 2009-10 school year. Please print off the page/schedule you need and drop it in the extended day box located by the parent mailboxes in the hallway. Thank you!
RE-ENROLLMENT FORMS
2010-11 RE ENROLLMENT BEGINS FEB. 22nd.
Click on this link to download both the Change to Student & Family Information Form and the Permission Form. If you have NO CHANGES to your contact, emergency, or pick up list information you will only need to fill out the Permission Form for re-enrollment for the 2010-11 school year and sign the information sheet in your parent mailbox. If any of your information has changed please fill out the areas that apply on the Change of Student & Family Information. Turn everything into the office and you are done.
Once you have turned in the necessary paperwork, your child(ren) will be officially enrolled and your 2010-11 contract will be generated and put in your box. Please sign and return your contract to the office.
YOU MUST FILL OUT THE PERMISSION FORM EVERY YEAR.
