APPLICATION FOR ADMISSION
If you are just beginning the admissions process and you would like to apply, please download this form and mail it to: Corvallis Waldorf School, 3855 NE Highway 20, Corvallis, OR 97330 or email it to: enrollment@corvalliswaldorfschool.org. A $50 admissions fee is required in order to process your application.
ENROLLMENT FORM
If you have completed the first stages of admission, have had a meeting with your teacher(s), and have been accepted into the class then you will need to fill out this form. All enrollment paperwork must be complete and in the office before your start date.
